Wound Wise Clinic LLC provides specialized wound care and related medical services including:

• Wound assessment and treatment
• Wound debridement and dressing changes
• Compression therapy
• Patient education and home care instructions
• Coordination with other healthcare providers
• Follow-up care and monitoring

Our services are limited to wound care and related specialties. We do not provide:

• Emergency medical services
• Primary care services
• Surgery requiring hospital facilities
• Services outside our area of expertise

• Patients arriving more than 15 minutes late may need to reschedule
• Chronic lateness may result in scheduling restrictions

• Payment is due at the time of service unless other arrangements are made
• You are responsible for all charges not covered by insurance
• We accept cash, credit cards, and approved payment plans

• We will verify insurance benefits but cannot guarantee coverage

If any provision of these terms is found to be unenforceable, the remaining provisions will continue in full force and effect.
